(“PBO”) Parkersburg, West VirginiaA company that provides medical practice management and administrative services to healthcare providers became aware of a data security incident that could affect the protected health information of certain individuals. The PBO has notified potentially affected individuals of the incident and provided resources to assist them.
exist April 2022, PBOs became aware of unusual activity in their network environment. The PBO took immediate steps to protect its network and hired an independent digital forensics and incident response firm to assist. The PBO determined that certain information stored on its network was accessed during the incident and may have been obtained without authorization, including certain protected health information maintained by the PBO in the course of its services. The PBO then works to review the potentially affected data to identify potentially affected individuals and the healthcare providers who maintain the information on their behalf. PBO completes these efforts June 30, 2022 Provide incident notification to relevant healthcare providers July 26, 2022. After coordinating notices with suppliers, PBOs endeavour to collect the current mailing addresses of all potentially affected individuals. PBO completes the process September 16, 2022 and arrange for a notification letter to be issued as soon as possible thereafter.United Nations September 23, 2022provides notice to all potentially affected individuals and resources to assist them, including supplemental credit monitoring and identity protection services.
Incidents may involve the following protected health information: name, home address, date of birth, social security number, driver’s license number, medical and diagnostic information, disability codes, prescription information, and health insurance account information. PBO has no evidence that any such information has been misused.
PBOs take the security of their information very seriously and have taken steps to help prevent similar incidents in the future.
